What we collect

We collect only the information you choose to send us through the pilot request form or the contact form: your name, organization, job title, work email, optional phone number, city, state, optional ZIP, organization type, number of locations, the interests you select, and any message you write.

We also record the page a submission came from and any campaign parameters (such as utm_source) present in the link you arrived through, so we can understand which conversations came from where.

What we do not want

Please do not send patient names, health information, or any protected health information through these forms. This website is not a secure channel for patient data.

How we use it

We use your information to reply to you, evaluate pilot requests, and communicate about the program. We do not sell your information.

How it is stored

Submissions are stored in a restricted database that is not readable from this website. Access is limited to An Honest Voyage. Submissions may be relayed to internal tools we use to manage follow-up.

Analytics

The site is built to support privacy-respecting analytics events (for example, that a form was submitted). Analytics events do not include the contents of your message.

Your choices

You can ask us to correct or delete the information you submitted by emailing Info@anhonestvoyage.com.
